On 03/12/2008, Boston Scientific SA has withdrawn from the market the batches listed in the appendix to the reference letter M004218400 of the medical device called: Ablation ™ radio frequency ablation ablation system pedals, manufactured by Boston Scientific Corporation. Indeed, the examination of a device, returned as a result of a complaint from a customer, revealed a problem of short circuit at the connector of the pedal. This defect can cause the emission of energy in the form of radiofrequency without pressing the pedal. If the ablation catheter has already been introduced into the patient, there is a risk of inadvertent ablation of cardiac or vascular tissue, which may cause bleeding and possibly perforate the heart tissue and result in tamponade (tamponade is defined as the excessive presence of fluid in the pericardium, preventing the heart from contracting normally). Boston Scientific SA has directly notified the recipients of the incriminated lots with the attached message (04/12/2008) (1093 ko) validated by Afssaps. This information is addressed to the directors and correspondents of materiovigilance for diffusion if necessary to the services concerned. The relevant European Competent Authorities are informed of this measure by the manufacturer.

“We take a patient-first approach to assessing the applicability of every recall and communicate to regulatory bodies in all geographies where the recalled device is sold,” Boston Scientific said in a statement to ICIJ. “We have coordinated several recalls across many countries in a timely manner,” the company said, adding that it complies with all national laws, which can often vary and require different processes for reporting information or taking action on recalls. The company said it uses a rigorous and uniform process to take action on recalls and that “when we initiate a field action (e.g. recall, safety alert), every customer who has received an affected product receives a communication that includes a letter for the physician.”
